Life! Helping you prevent Diabetes and Cardiovascular Disease
What is the Life! Program?
The Life! Program will support you to take control of your life, teaching you how to adopt healthy behaviours and a more active lifestyle to reduce your risk of Type 2 Diabetes and heart disease. Funded by the Victorian Government and managed by Diabetes Australia - Victoria, The Life! Program runs over six sessions (one individual and five group sessions) and is delivered by expert allied health professionals including Accredited Exercise Physiologists, Accredited Practicing Dietitians, Registered Nutritionists, Credentialled Diabetes Educators and Nurses.
Our Senior Exercise Physiologist, Nicole French, has been facilitating programs since the initial roll out of the program in Victoria in 2008. Through her efforts she and her team have assisted more than 1000 individuals to take control of their lifestyles through healthy eating, exercise and goal-setting, to empower them to make sustainable changes that help to prevent chronic disease.
About the Life! Program
The Life! group course provides one individual and five valuable group sessions over a six month period. Group sessions 1-4 run fortnightly where you will develop the confidence to make small, achievable changes to improve your lifestyle and learn problem solving skills. Group members are encouraged to reach a weight loss goal through healthy eating and moderate daily exercise. Group session 5 takes place four months after session 4 and acts as a 'catch up' or 'review' session. Upon completion, group members are linked into local support networks (such as exercise groups) to allow them to maintain their improved lifestyle. Although it's in a group environment, we still do offer an individualised approach - our team will work with you to create a customised plan to change your lifestyle to reduce your risk of type 2 diabetes and cardiovascular disease. New Life! Programs begin every 6-8 weeks at our clinic and are scheduled on weeknights and weekends to ensure greater access for all.
Eligibility for the program
To participate in the Life! program, you must have one or more of the following:
- 18 years and over and have an AUSDRISK score≥ 12 and a BMI score ≥ 25
- 18 years or more and have previously been diagnosed with one or more of the following:
- cardiovascular disease (including myocardial infarction, angina and angioplasty)
- gestational diabetes
- moderate or severe Chronic Kidney Disease
- familial hypercholesterolemia
- serum total cholesterol >7.5mmol/L
- systolic BP of ≥ 180mmHg or diastolic BP of ≥ 110mmHg
- impaired glucose tolerance (by FPG, OGTT or HbA1c)
- polycystic ovary syndrome
- 45 years or more, or ≥ 35 years and of Aboriginal and/or Torres Strait Islander descent, and have an Absolute Risk score of ≥10% when referred by a GP clinic
To be eligible for the program you MUST NOT HAVE Diabetes, clinically active cancer and must not be pregnant.

Prior to attending the group program you will attend an introductory session with one of our team members. If you fulfill the eligibility requirements and would like to commence a Life! Program or book in for an introductory session, please contact us.
Please note - We do not charge you for the introductory session or the Life! Program. That's right, it's FREE! And you don't need a doctors referral to be involved, however, you will need to obtain a copy of your most recent blood tests (must not be greater than 12 months old) that detail your cholesterol and blood glucose.
