    Referral requirements

Before attending an appointment you will need to plan a visit to your General Practitioner to obtain a referral to see the Exercise Physiologist. The referral may either be on a D904 slip or on your doctors letterhead - be sure to advise your doctor to provide as much clinical information as possible on the referral so that we can help you best. After your initial assessment, your doctor will receive a report from your treating Exercise Physiologist outlining your goals and treatment plan.

For more information regarding eligibility and referral requirements please visit the DVA website or speak with your General Practitioner.
